Job Summary
Coordinate, implement, and evaluate home care needs for patients recovering from illness, injury, or surgery by creating and updating comprehensive, interdisciplinary care plans. Manage medication, conduct patient assessments, and provide education while reviewing insurance information and sharing findings with families and professional caregivers. Case manage services to ensure positive clinical outcomes and minimize rehospitalization risks while supporting agency financial goals. Requires current NYS RN license, acute medical-surgical experience, and strong independent problem-solving skills.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
- Excellent communication, documentation and time management capabilities
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
