Coordinate, implement, and evaluate home care needs for patients recovering from illness, injury, or surgery. Create initial comprehensive, interdisciplinary patient-centered home health plans of care and update them in collaboration with physicians. Review patient insurance information, coordinate individual care plan activities, and ensure pertinent findings are shared with family and professional caregivers. Provide advanced clinical services including patient assessment, medication management, and patient/family education while effectively case managing care to minimize rehospitalization risks. Support agency financial goals and demonstrate expert community health nursing knowledge with strong independent problem-solving and team interaction skills. Current NYS RN license, Bachelor's degree, and one year acute medical-surgical experience required.
