Coordinate, implement, and evaluate the home care needs of patients recovering from illness, injury, or surgery at home. Create initial comprehensive interdisciplinary patient-centered home health plans of care and update them in collaboration with physicians. Manage medication administration, conduct patient assessments, and provide family education while reviewing insurance information to coordinate individual care activities. Share pertinent findings with family and professional caregivers to ensure safe transitions and minimize rehospitalization risks. Support agency financial goals through effective case management, documentation, and time management. Requires current NYS RN license, acute medical-surgical experience, and bilingual English/Spanish proficiency.
