Job Summary
Coordinate and oversee the home care needs of patients recovering from illness, injury, or surgery by creating initial comprehensive interdisciplinary care plans and updating them in collaboration with physicians. Provide advanced clinical services including patient assessment, medication management, and patient/family education while reviewing insurance information and coordinating activities within individual care plans. Share pertinent findings with family and professional caregivers to ensure effective case management that minimizes rehospitalization risks and supports agency financial goals. This role requires a current NYS RN license, one year of acute medical-surgical experience, and CHHA experience preferred, with a driver's license and car required. Bilingual English/Spanish skills are an asset.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
- Bilingual English / Spanish preferred
