Job Summary
Coordinate the initial comprehensive and interdisciplinary patient-centered home health plan of care, updating it in collaboration with physicians. Implement and continually evaluate home care needs for patients recovering from illness, injury, or surgery at home. Review patient insurance information, coordinate individual care plan activities, and share pertinent findings with family and professional caregivers. Manage medication, conduct patient assessments, and provide family education to ensure positive clinical outcomes and minimized rehospitalization risks. Demonstrate expert community health nursing knowledge and solve problems independently while supporting agency financial goals. Requires current NYS RN license, Bachelor's degree preferred, and one year acute medical-surgical nursing experience.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
- Excellent communication, documentation and time management capabilities
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
