Job Summary
Coordinate, implement, and evaluate home care needs for patients recovering from illness, injury, or surgery. Create initial comprehensive interdisciplinary patient-centered care plans and update them in collaboration with physicians. Review patient insurance information, coordinate individual care plan activities, and share pertinent findings with family and professional caregivers. Provide advanced clinical services including patient assessment, medication management, and patient/family education. Effectively case manage care to ensure positive clinical outcomes and minimize rehospitalization risks while supporting agency financial goals. Requires current NYS RN license, Bachelor's degree, acute medical-surgical experience, and CHHA preference.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
