Job Summary
Coordinate and oversee the home care needs of patients recovering from illness, injury, or surgery by creating initial interdisciplinary care plans and updating them in collaboration with physicians. Manage medication, conduct patient assessments, and provide education while reviewing insurance information to ensure findings are shared with families and professional caregivers. Effectively case manage assigned patients to achieve positive clinical outcomes and minimize rehospitalization risks while supporting agency financial goals. Requires current NYS RN license, acute medical-surgical experience, and bilingual English/Spanish skills.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
- Bilingual English / Spanish preferred
