Job Summary
Coordinate and implement comprehensive home care plans for patients recovering from illness, injury, or surgery, including patient assessment, medication management, and family education. Manage assigned cases to ensure positive clinical outcomes and minimize rehospitalization risks while reviewing insurance information and sharing findings with caregivers. Continually evaluate home health needs, update care plans in collaboration with physicians, and support agency financial goals through expert community health nursing knowledge. Maintain strong documentation, time management, and independent problem-solving skills within an integrated team. Requires current NYS RN license, acute medical-surgical experience, and CHHA preference.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
- Bilingual English / Spanish preferred
