Job Summary
Coordinate and oversee the home care needs of patients recovering from illness, injury, or surgery by creating initial comprehensive interdisciplinary care plans and updating them in collaboration with physicians. Manage medication administration, conduct patient assessments, and provide family education while ensuring pertinent findings are shared with professional caregivers. Effectively case manage assigned patients to achieve positive clinical outcomes and minimize rehospitalization risks, supporting agency financial goals through expert community health nursing knowledge. Maintain accurate documentation, review insurance information, and independently solve clinical problems within an integrated team environment. Requires current NYS RN license, Bachelor's degree, acute medical-surgical experience, and bilingual English/Spanish skills.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team

Desired Qualifications
