Coordinate, implement, and continually evaluate the home care needs of patients recovering from illness, injury, or surgery at home. Provide advanced clinical services including patient assessment, medication management, and patient/family education. Create initial comprehensive interdisciplinary home health plans of care and update them in collaboration with physicians. Review patient insurance information, coordinate individual care plan activities, and ensure pertinent findings are shared with family and professional caregivers. Effectively case manage care and services for assigned patients to achieve positive clinical outcomes and minimize rehospitalization risks while supporting agency financial goals. Requires current NYS RN license, Bachelor's degree in nursing, and minimum one year acute medical-surgical nursing experience.
