Job Summary
Map out and oversee patient recovery processes from illness, injury, or surgery at home by providing advanced clinical services including patient assessment, medication management, and education. Coordinate, implement, and continually evaluate individual home care needs while reviewing insurance information and ensuring findings are shared with family and professional caregivers. Create initial comprehensive interdisciplinary patient-centered home health plans of care and update them in collaboration with physicians to minimize rehospitalization risks. Effectively case manage assigned patients to support agency financial goals while demonstrating expert community health nursing knowledge and independent problem-solving skills. Requires current NYS RN license, Bachelor's degree preferred, and one year acute medical-surgical experience.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
- Bilingual English / Spanish preferred
