Job Summary
Coordinate and implement comprehensive home health plans of care for patients recovering from illness, injury, or surgery. Conduct patient assessments, manage medications, and provide family education while reviewing insurance information and sharing findings with caregivers. Effectively case manage services to ensure positive clinical outcomes and minimize rehospitalization risks. Collaborate with physicians to update care plans and support agency financial goals through expert community health nursing practice. Requires current NYS RN license, acute medical-surgical experience, and a driver's license.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
