Coordinate, implement, and evaluate the home care needs of patients recovering from illness, injury, or surgery at home. Create initial comprehensive, interdisciplinary patient-centered home health plans of care and update them in collaboration with physicians. Manage medication, conduct patient assessments, and provide education to patients and families. Review insurance information, coordinate individual care plan activities, and ensure pertinent findings are shared with professional caregivers. Case manage care and services to achieve positive clinical outcomes and minimize rehospitalization risks while supporting agency financial goals. Requires current NYS RN license, Bachelor's degree, and one year acute medical-surgical nursing experience.
