Job Summary
Coordinate, implement, and evaluate the home care needs of patients recovering from illness, injury, or surgery at home. Create initial comprehensive interdisciplinary home health plans of care and update them in collaboration with physicians. Effectively case manage services for assigned patients to ensure positive clinical outcomes and minimize rehospitalization risks. Review patient insurance information, coordinate individual care plan activities, and share pertinent findings with families and professional caregivers. Provide advanced clinical services including patient assessment, medication management, and patient/family education while supporting agency financial goals. Requires current NYS RN license, driver's license, and car; bilingual English/Spanish preferred.
Required Qualifications
- Current NYS RN license and registration
- Minimum of one year acute medical-surgical nursing experience
- Driver license and car
- Demonstrate expert knowledge unique to the practice of community health nursing
- Excellent clinical assessment skills
- Strong ability to solve problems independently and interact with an integrated team
Desired Qualifications
- Bachelor's degree in nursing preferred
- CHHA experience preferred
- Bilingual English / Spanish preferred
