Coordinate and implement comprehensive home care plans for patients recovering from illness, injury, or surgery, conducting patient assessments, managing medications, and providing family education. Oversee the full continuum of care by reviewing insurance information, coordinating interdisciplinary activities, and ensuring findings are shared with professional caregivers. Create initial patient-centered care plans and update them in collaboration with physicians to minimize rehospitalization risks and support agency financial goals. Demonstrate expert clinical assessment skills and solve problems independently while maintaining strong documentation and communication with the integrated team. Requires current NYS RN license, one year acute medical-surgical experience, and a driver's license.
