Coordinate, implement, and evaluate the home care needs of patients recovering from illness, injury, or surgery at home. Create initial comprehensive interdisciplinary patient-centered home health plans of care and update them in collaboration with physicians. Manage medication, conduct patient assessments, and provide family education while reviewing insurance information and coordinating activities for each individual care plan. Share pertinent findings with family and professional caregivers to ensure positive clinical outcomes and minimize rehospitalization risks. Demonstrate expert knowledge of community health nursing, solve problems independently, and support agency financial goals through effective case management. Requires current NYS RN license, Bachelor's degree, and one year acute medical-surgical nursing experience.
