Job Summary
Conduct enterprise and local strategies to close STARS gaps and perform medical record reviews to document Focus On Care measures. Request records, upload documentation to the electronic system, and execute patient outreach calls to support quality gap closure, including assisting members with scheduling screenings. Schedule ancillary visits for in-center screenings and meet regularly with Market Leaders to discuss roadblocks and strategize initiatives. Ensure compliance with OSHA safety standards and maintain accurate, legal, and ethical documentation while adhering to HIPAA confidentiality. Requires working knowledge of CMS STARS/HEDIS, primary care experience, and proficiency with Microsoft Office Suite.
Required Qualifications
- High school diploma or equivalent education (GED)
- A minimum of 1 year work experience as a Medical Assistant
- Graduation from a nationally accredited Medical Assistant program OR comparable healthcare vocational training (i.e, foreign-trained physician, paramedic, etc.)
- BLS for Healthcare Providers
- EClinicalWorks or similar EMR experience
- Working knowledge of quality measures and quality gap closure strategies
- Experience working in a primary care setting
- Proficient with Microsoft Office Suite products including Word, Excel, Power Point, Outlook, and Teams
- Demonstrated team building skills
- Ability to work independently with a high level of attention to detail
- Sensitivity to the needs of culturally diverse staff, patients, and communities
- Maintains HIPAA standards and confidentiality of protected health information; reports critical incidents immediately
Desired Qualifications
- Experience working with geriatric patients
- Experience with the CMS STARS program (HEDIS)
