Job Summary
Conduct Family Needs Assessments using standardized tools and develop action plans based on findings. Coordinate care planning meetings, schedule evening and weekend sessions, and follow up with families to monitor progress and revise care plans as needed. Assist families in setting goals within their Family Care Plan and understanding options within the care system. Provide health needs coordination, document all encounters in permanent health records, and ensure adherence to organizational standards and training requirements.
Required Qualifications
- Bachelor's degree preferred
- Experience working children and families in the community from diverse populations
- Experience working with children and families with special health care needs preferred
- Bilingual preferred
- Rhode Island Community Health Worker Certification preferred; certification required within the first 12 months of employment
- Excellent communication skills
- Ability to utilize an Electronic Medical Record and familiarity with Outlook, Word, Excel
- Ability to work in a hybrid environment while maintaining organizational PHI standards
- Reliable transportation, current driver's license, proof of automobile insurance and automobile registration
- Ability to lift 20 pounds
- Ability to communicate effectively
