Job Summary
Coordinated Housing Access Team (CHAT) Case Manager, based in Portland, OR, provides outreach and engagement to people experiencing homelessness, administers VI-SPDAT and VAT tools for housing prioritization, assists with housing placement documentation, participates in Coordinated Access activities and case conferencing, builds relationships with community partners, maintains participant files and HMIS documentation, and supports monthly reporting. Requires trauma-informed outreach, strong coordination skills, and the ability to work mobile across the county; bachelor’s degree or equivalent social services experience is required, with fluency in English/Spanish preferred and valid driver’s license. Union-represented, full-time position with benefits.
Required Qualifications
- Bachelor's Degree in a related field OR 2 years of experience in social services required.
- Demonstrated ability to interact with other agencies in developing and providing services to participants; including ability to advocate on behalf of participants for services with other agencies.
- Demonstrated knowledge of chemical dependency and mental health issues and appropriate treatment resources.
- Demonstrated ability to serve people competently and effectively from historically underserved communities, with specific focus on communities of color.
- Ability to identify and work effectively with property and building managers, developing positive long-term relationships.
- Demonstrated ability to work as part of a team in the delivery of services to participants.
- Computer literacy. Demonstrated ability to use word processing, database, spreadsheet, e-mail, and Internet programs.
- Demonstrated ability to maintain accurate and comprehensive participant files.
- Demonstrated ability to prepare accurate and comprehensive statistical and narrative reports.
