Job Summary
Coordinate communications with the outpatient transplant population for clinic visits, community referrals, telephone consultations, long-range beepers, outpatient treatments, and hospital admissions. Develop and maintain nursing practice standards of care while identifying and resolving clinical issues, including reporting signs of possible rejection immediately to the physician. Collaborate with staff, patients, and families on evaluation and discharge planning, and participate in daily patient rounds and interdisciplinary research activities. Maintain appropriate documentation for candidacy and post-transplant follow-up, ensuring protocol medications and diagnostic tests are ordered and reviewed. Greets customers using the EHC model and participates in Press Ganey patient satisfaction initiatives. Requires an Associate's degree in nursing, three years of clinical experience, and an active nursing license.
Required Qualifications
- An Associate's degree in nursing
- 3 years of recent, clinical, medical-surgical nursing experience
- A valid, active license or temporary permit approved by the Georgia Licensing Board
- BLS certification
- An active compact/multistate license (eNLC) within 60 days of hire (for employees completing virtual care activities that may include multi-state practice)
- Up to 25 lbs. lifting (0-33% of the work day, occasionally)
- Carrying of objects up to 25 lbs.
- Occasional to frequent standing & walking
- Occasional sitting
- Close eye work (computers, typing, reading, writing)
- Shift work
- Travel may be required
