Job Summary
Conduct in-home assessments and functional evaluations for older adults and individuals with disabilities. Deliver personalized, evidence-based recommendations for home modifications and educate clients and caregivers on fall prevention and safe mobility techniques. Engage families in collaborative care planning while maintaining accurate documentation through our electronic system. Work closely with the Director of Clinical Operations to share findings and support process improvement initiatives. This contracted role requires a minimum of 10 hours of weekly availability with an anticipated start date of July 15, 2026. Compensation is $120 per completed evaluation, covering travel, the evaluation itself, and documentation.
Required Qualifications
- Degree in Occupational Therapy from an accredited program
- Active, unencumbered OT license
- 2+ years of clinical experience
- Reliable transportation and valid driver's license
- Proficiency with technology and digital documentation
- Ability to work independently while collaborating with a remote team
