Job Summary
Conduct in-home assessments and functional evaluations for older adults and individuals with disabilities, delivering personalized, evidence-based recommendations for home modifications. Educate clients and caregivers on fall prevention and safe mobility techniques while engaging families in collaborative care planning. Work closely with the Director of Clinical Operations to share findings and support process improvement initiatives, maintaining accurate documentation through the electronic system. Earn $120 per completed evaluation with a minimum of 10 hours of weekly availability starting August 1, 2026. This role supports Jukebox Health's mission to empower independent living at home through technology-driven home modifications and environmental supports nationwide.
Required Qualifications
- Degree in Occupational Therapy from an accredited program
- Active, unencumbered OT license
- 2+ years of clinical experience
- Reliable transportation and valid driver's license
- Proficiency with technology and digital documentation
- Ability to work independently while collaborating with a remote team
