Job Summary

Conduct patient care using the nursing process, assess learning needs, and implement teaching strategies for patients, families, and disciplines. Manage nursing team resources, communicate with the interdisciplinary group, and adapt communication styles to specific patient populations while respecting privacy. Evaluate outcomes using data to promote change and ensure compliance with federal, state, and local laws, accreditation standards, and the Compliance & Privacy Program. Perform duties involving exposure to blood, body fluids, and hazardous medications while maintaining self-directed organizational skills. Requires a current RN license, BLS certification, and at least one year of home health experience; includes 5x 8-hour shifts Monday through Friday.

Required Qualifications

  • RN- Registered Nurse
  • License of State in which intend to work or Compact License
  • >1 year experience required
  • Local or travel accepted
  • Required - Registered nurse degree
  • Valid Driver License and Auto Insurance
  • Minimum 1 year experience in Home Health setting as RN
  • HCHB EMR experience
  • OASIS start of care experience
  • Required - Current registered nurse (RN) license in state of practice
  • Current Basic Life Support (BLS) certification from the American Heart Association
  • Proficiency in using computers, software, and web-based applications
  • Effective verbal and written communication skills and ability to present information clearly and professionally
  • Strong interpersonal skills
  • Good organizational and time management skills and ability to be self-directed
  • Ability to demonstrate good judgement
  • Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects
  • Duties performed routinely require exposure to blood, body fluid and tissue
  • The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain communicable diseases
  • Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste

Desired Qualifications

  • >2 year experience preferred
  • Preferred - Associate or bachelor's degree in nursing
