Job Summary
Plan patient movement through the healthcare system by managing care aspects, applying evidence-based criteria, and reinforcing post-discharge care plans. Direct licensed and ancillary team members while maintaining accountability for quality outcomes and adapting communication styles to specific patient populations. Ensure compliance with federal, state, and local laws, accreditation standards, and the Compliance & Privacy Program, including immediate reporting of any safety or privacy concerns. Work 5x 8h shifts Monday through Friday. Requires a Registered Nurse Diploma, current RN license, BLS certification, and two years of related experience.
Required Qualifications
- Registered Nurse Diploma
- 2 Years of Related Experience
- Current RN License in the state of practice
- Current Basic Life Support (BLS) certification from the American Heart Association
Desired Qualifications
- Associate's or Bachelor's degree in Nursing
- Certification in Case Management (CCM)
- Certification in Health Care Management (CPHM)
