Job Summary
Assess patient learning needs, implement tailored teaching strategies, and manage nursing team resources while delivering care in the home health field. Utilize the nursing process to evaluate patient outcomes, adapt communication styles to diverse populations, and ensure compliance with federal, state, and local regulatory standards. Maintain proficiency in HCHB EMR and OASIS start-of-care documentation, demonstrate strong interpersonal and organizational skills, and work a 5x 8-hour schedule Monday through Friday. Requires current RN license, BLS certification, and at least one year of home health experience.
Required Qualifications
- Registered nurse degree
- Valid Driver License and Auto Insurance
- Minimum 1 year experience in Home Health setting as RN
- HCHB EMR experience
- OASIS start of care experience
- Current registered nurse (RN) license in state of practice
- Current Basic Life Support (BLS) certification from the American Heart Association
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally
- Strong interpersonal skills
- Good organizational and time management skills and ability to be self-directed
- Ability to demonstrate good judgement
- RN - Registered Nurse
- License of State in which intend to work or Compact License
- >1 year experience required
- Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly
- Duties performed routinely require exposure to blood, body fluid and tissue
- Occupational risk for exposure to communicable diseases
- Occupational risk for exposure to hazardous medications or hazardous waste
Desired Qualifications
- Associate or bachelor's degree in nursing
- >2 year experience preferred
- Local or travel accepted
