Job Summary
Assess patient needs and implement teaching strategies for diverse groups, while effectively using the nursing process to deliver care. Manage nursing team resources and communicate clearly with patients, families, and the interdisciplinary team. Adapt communication styles to specific patient populations and ensure compliance with federal, state, and local laws and accreditation standards. Maintain proficiency in HCHB EMR and OASIS documentation, including start-of-care assessments. Perform duties requiring lifting up to 100 pounds and exposure to blood or hazardous medications. Work a 5x 8-hour schedule Monday through Friday.
Required Qualifications
- Registered nurse degree
- Valid Driver License and Auto Insurance
- Minimum 1 year experience in Home Health setting as RN
- HCHB EMR experience
- OASIS start of care experience
- Current registered nurse (RN) license in state of practice
- Current Basic Life Support (BLS) certification from the American Heart Association
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally
- Strong interpersonal skills
- Good organizational and time management skills and ability to be self-directed
- Ability to demonstrate good judgement
- RN - Registered Nurse
- License of State in which intend to work or Compact License
- >1 year experience required
- Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly
- Duties performed routinely require exposure to blood, body fluid and tissue
- Works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain communicable diseases
- Occupational risk for exposure to communicable diseases
- Occupational risk for exposure to hazardous medications or hazardous waste
Desired Qualifications
- Associate or bachelor's degree in nursing
- >2 year experience preferred
- Local or travel accepted
