Job Summary

Conduct home health visits to deliver patient care using the nursing process, assessing learning needs and implementing tailored teaching strategies for patients, families, and interdisciplinary teams. Manage nursing resources effectively while adapting communication styles and procedures to diverse patient populations, ensuring compliance with federal, state, and local laws and accreditation standards. Maintain knowledge of current regulatory requirements and safety protocols, including exposure risks to blood, body fluids, and hazardous medications. Complete assignments in a 5x 8-hour schedule Monday through Friday, requiring a current RN license, at least one year of home health experience, and proficiency with HCHB EMR.

Required Qualifications

  • RN - Registered Nurse
  • License of State in which intend to work or Compact License
  • >1 year experience required
  • Local or travel accepted
  • Required - Registered nurse degree
  • Valid Driver License and Auto Insurance
  • Minimum 1 year experience in Home Health setting as RN
  • HCHB EMR experience
  • Required - Current registered nurse (RN) license in state of practice
  • Current Basic Life Support (BLS) certification from the American Heart Association
  • Proficiency in using computers, software, and web-based applications
  • Effective verbal and written communication skills and ability to present information clearly and professionally
  • Strong interpersonal skills
  • Good organizational and time management skills and ability to be self-directed
  • Ability to demonstrate good judgement
  • Must be able to lift 50 to 100 pounds of force occasionally
  • Must be able to lift 25 to 50 pounds of force frequently
  • Must be able to lift 10 to 20 pounds of force constantly
  • Must be able to work in an area where patients enter
  • Must be able to work directly with patients
  • Must be able to work with specimens that could contain communicable diseases
  • Must be able to handle receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste

Desired Qualifications

  • >2 year experience preferred
  • Preferred - Associate or bachelor's degree in nursing
