Job Summary
Support consumers transitioning from nursing homes under the Money Follows the Person Program by providing information, referrals, and advocacy while assisting with community needs and independent living plans. Work directly with clients to identify barriers, update goals, and coordinate resources through the IL and TC coordinators. Complete all required documentation in the mandated database and submit invoices for the 200-hour support period at a rate of $12.50 per hour. Applicants must have disability experience, with a preference for applicants with disabilities, and possess skills in communication, documentation, and resource management.

- ability to submit an invoice for hours worked to the director as requested
