Job Summary
Conduct home health visits for patient care, assessing learning needs and implementing teaching strategies for patients, families, and diverse groups. Manage nursing team resources and communicate effectively within the interdisciplinary team while adapting behavior to specific patient populations regarding privacy and communication styles. Perform related duties as required, ensuring compliance with federal, state, and local laws, accreditation standards, and the employer's Compliance & Privacy Program. Work 5x 8h shifts Monday through Friday. Maintain a valid driver's license, auto insurance, and HCHB EMR experience.
Required Qualifications
- LPN
- License of State in which intend to work or Compact License
- >1 year experience required
- Local or travel accepted
- Required - LPN Diploma
- Valid Driver License and Auto Insurance
- Minimum 1 year experience in Home Health setting as LPN
- HCHB EMR experience
- CPR / BLS
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally
- Strong interpersonal skills
- Good organizational and time management skills and ability to be self-directed
- Ability to demonstrate good judgement
