Job Summary
Deliver patient care at home using the nursing process, assessing learning needs, and implementing teaching strategies for patients, families, and diverse groups. Communicate with and manage nursing team resources while adapting communication styles to specific patient populations and respecting privacy. Perform home visits requiring lifting up to 100 pounds, with routine exposure to blood, body fluids, and hazardous medications. Maintain compliance with federal, state, and local laws, accreditation standards, and the employer's Compliance & Privacy Program. Requires an active LPN license, one year of home health experience, HCHB EMR proficiency, and valid driver's license. Health insurance available upon assignment start.
Required Qualifications
- LPN
- License of State in which intend to work or Compact License
- >1 year experience required
- Local or travel accepted
- Required - LPN Diploma
- Valid Driver License and Auto Insurance
- Minimum 1 year experience in Home Health setting as LPN
- HCHB EMR experience
- CPR / BLS
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally
- Strong interpersonal skills
- Good organizational and time management skills and ability to be self-directed
- Ability to demonstrate good judgement
