Job Summary
Conduct home visits to deliver patient care using the nursing process, assess learning needs, and implement teaching strategies for patients, families, and diverse groups. Manage nursing team resources, communicate with the interdisciplinary team, and adapt communication styles to specific patient populations while respecting privacy. Perform related duties including lifting up to 100 pounds occasionally and managing exposure to blood, body fluids, and hazardous medications. Maintain knowledge of federal, state, and local laws, accreditation standards, and compliance programs. Requires an active LPN license, minimum one year of home health experience, HCHB EMR proficiency, and valid driver's license. Offers health insurance upon assignment start.
Required Qualifications
- LPN
- License of State in which intend to work or Compact License
- >1 year experience required
- Local or travel accepted
- Required - LPN Diploma
- Valid Driver License and Auto Insurance
- Minimum 1 year experience in Home Health setting as LPN
- HCHB EMR experience
- CPR / BLS
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally
- Strong interpersonal skills
- Good organizational and time management skills and ability to be self-directed
- Ability to demonstrate good judgement
