Job Summary
Deliver patient care in home health visits by assessing learning needs, implementing teaching strategies, and adapting communication styles to diverse patient populations. Manage nursing team resources, communicate effectively with interdisciplinary partners, and ensure compliance with federal, state, and local laws while maintaining patient privacy. Perform duties requiring physical exertion of up to 100 pounds and manage occupational risks for bloodborne pathogens and hazardous medications. Work a 5x 8-hour schedule Monday through Friday with a valid driver's license, HCHB EMR experience, and 1+ year of home health LPN experience.
Required Qualifications
- LPN
- License of State in which intend to work or Compact License
- >1 year experience required
- Local or travel accepted
- Required - LPN Diploma
- Valid Driver License and Auto Insurance
- Minimum 1 year experience in Home Health setting as LPN
- HCHB EMR experience
- CPR / BLS
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally
- Strong interpersonal skills
- Good organizational and time management skills and ability to be self-directed
- Ability to demonstrate good judgement
