Job Summary
Provide skilled nursing visits to patients in their homes and community settings, including nursing care, diagnostic procedures, and lab tests. Monitor patient care and conduct nurse supervisory visits with caregivers as needed. Liaise between the office, caregivers, and other healthcare professionals. Conduct assessments, admissions, and care planning for new patients. Collaborate with patients, families, and caregivers to develop individualized care plans.
Required Qualifications
- Graduate of an accredited school of professional nursing
- At least two years of general nursing experience (home care, medical, surgical, or critical experience)
- Complies with accepted professional standards and principles
- Possesses and maintains good physical stamina and mental health
- Pre-employment physician health clearance with negative TB skin test
- Current First Aid & CPR certification
- Licensed driver with an insured automobile
- Excellent references from previous/current employers and/or professional peers
- Ability to work independently, multi-task, and meet deadlines
