Job Summary
Facilitate receiving medical records, review patient history, and complete consents for patients in Irmo, Columbia, and Blythewood. Initiate Care Management plans, capture diagnoses, and perform cognitive impairment screenings and Social Determinants of Health assessments. Coordinate care by reconciling medications, evaluating for home health or hospice needs, and providing post-discharge education. Document all Care Management activities and update care plans in conjunction with the interdisciplinary team. This full-time, salary-based role requires a licensed nurse with at least three years of clinical experience, reliable transportation, and the ability to work 12-hour PM shifts on a 7-on/7-off schedule.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
