Job Summary
Coordinate continuity of care for individuals transitioning from State Mental Health Facilities, Private Psychiatric Beds, or Residential Treatment Centers by developing individualized discharge plans and ensuring uninterrupted service delivery through community reintegration. Assess medical, psychiatric, and rehabilitative needs while communicating clinical status with facilities and attending treatment team meetings to prevent service gaps. Monitor admission and discharge compliance with Texas Administrative Code and HHSC requirements, scheduling psychiatric follow-up appointments within seven to fourteen days of discharge and coordinating access to housing, food, and educational supports. Maintain accurate clinical documentation in electronic health records and respond to challenging situations involving individuals exhibiting escalated behavior.
Required Qualifications
- Bachelor's degree from an accredited college or university in Social Work, Psychology, Counseling, Human Services, or a related field
- Two (2) years of experience in mental health services, case management, or care coordination
- Must have a valid drivers' license
- Regular attendance and reliability
- Ability to sit, stand, and walk for extended periods of time
- Ability to see, hear, and communicate effectively
- Ability to use a computer, telephone, and standard office equipment for prolonged periods
- Ability to perform repetitive tasks such as typing, filing, and data entry
- Ability to lift and/or move up to 15 pounds
- Ability to respond appropriately to challenging or unpredictable situations, including safely de-escalating and managing interactions with individuals who may exhibit verbally or physically escalated behavior
- Ability to perform the essential functions of the position safely and effectively, including maintaining professionalism, sound judgment, appropriate interpersonal interactions, concentration, and the ability to respond appropriately in stressful or crisis situations

- Experience coordination care across multiple systems (hospitals, community providers, etc.)
- Knowledge of community resources and behavioral health systems
