Job Summary
Continuing Care Coordinators manage client-related communications and coordination with referral sources (hospitals, outpatient practices, schools, and governmental organizations) and may discharge planning for clients. Key duties include providing treatment updates to referral providers, developing discharge plans, maintaining a nationwide provider database to identify referral options, making timely referrals to aftercare resources, following up to verify placement, documenting case management contacts, and collaborating with clinical and outreach teams to fulfill responsibilities. Requires a Bachelor’s degree in a related field, 2+ years of healthcare or client-facing experience, strong interpersonal and communication skills, proficiency with Salesforce and Google Suite, excellent organizational abilities, and US work authorization with English proficiency. This role is full-time, remote within the United States, with base pay between $52,500 and $60,000 plus potential incentives and benefits.
Required Qualifications
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- Minimum of 2 years of relevant work experience (e.g., healthcare, customer/patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach)
- Strong interpersonal, relationship-building and listening skills
- Metrics- and results-oriented mindset with experience working against concrete targets
- Excellent written and verbal communication skills
- Extreme organization and attention to detail
- Work authorized in the United States and native or bilingual English proficiency
- Proficient in Salesforce and Google Suite/MS Office
