Job Summary
Continuing Care Coordinators manage client-related communication and coordination with referral sources (hospitals, outpatient practices, schools, and governmental organizations). Responsibilities include providing treatment updates to referring providers, planning discharge for clients, maintaining a nationwide provider database to identify referral options, making referrals using external providers’ preferred communication channels, following up to verify placement, documenting case management contacts, and collaborating with clinical and outreach teams to fulfill responsibilities. Requires a Bachelor’s degree in health sciences, communications, psychology, social work, or related field and a minimum of 2 years of healthcare or customer-facing experience. Proficiency in Salesforce and Google Suite/MS Office, strong written and verbal communication skills, and the ability to thrive in a fast-paced environment. This is a full-time, exempt role with base pay between $52,500 and $60,000 per year, with additional compensation possible depending on location and experience; remote in the United States listed (#LI-REMOTE). Background checks are required for clearance to see clients, including state-specific fingerprint checks in Florida. Required Qualifications
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- Minimum of 2 years of relevant work experience in healthcare or customer/patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach
- Strong interpersonal, relationship-building and listening skills
- Metrics- and results-oriented mindset, with experience working against concrete targets
- Excellent written and verbal communication skills
- Extreme organization and attention to detail
- Work authorized in the United States and native or bilingual English proficiency
- Proficient in Salesforce and Google Suite/MS Office
