Job Summary
Provides information, advocacy, and service arrangement for consumers and families; participates in developing program plans and monitors/evaluates them; identifies and coordinates services; completes forms/reports; acts as an advocate for individuals served; schedules/interfiles interdisciplinary meetings with individuals, families, service providers, and advocates; identifies barriers to service delivery and applies culturally sensitive strategies; comfortable working in homes, communities, or office; rotates officer-of-the-day duties and provides on-call support when required; frequent meetings via telephone, virtual, and in-person; requires independence and ability to work with an interdisciplinary team.
Required Qualifications
- Bachelor’s degree in psychology, social work, sociology, or related human services field
- at least two years of related experience
- Master’s degree in a related subject may be substituted for experience
- Valid California Driver’s License and reliable transportation
- Bilingual Spanish may be required dependent upon position
