Job Summary
Provides information, advocacy, and service arrangement for consumers and families; participates in development of program plans; identifies and coordinates services; completes documentation and reports; serves as an advocate for individuals served by community agencies; schedules and attends interdisciplinary meetings; identifies barriers to service delivery and implements culturally sensitive strategies; rotates officer-of-the-day duties and provides emergency on-call services when required; role requires meeting with individuals in homes, community, or office, and is generally hybrid/remote with on-site elements; requires a Bachelor’s degree in a related field with at least two years of experience (Master’s degree may substitute), strong organizational and communication skills, independence and teamwork, knowledge of intellectual and developmental disabilities, and ability to work with diverse populations; valid CA driver's license is required; bilingual Spanish may be required depending on position; familiarity with MS Word and MS Outlook; effective in person-centered planning and empathetic service delivery.
Required Qualifications
- Bachelor’s degree in psychology, social work, sociology, or related human services field
- At least two years of related experience
- Valid California Driver’s License and reliable transportation
- Bilingual Spanish may be required dependent upon position
- Proficiency with MS Word and MS Outlook
