Consumer Services Coordinator (Case Management) for the Riverside East Transition Unit

Consumer Services Coordinator (Case Management) coordinates services for individuals with developmental disabilities and their families under the Lanterman Act, developing and executing IPPs, preparing progress reports, and conducting annual reviews. Responsibilities include advocacy, attending IEP/ITP/IHC and related meetings, coordinating assessments across medical, psychological, developmental, educational, vocational domains, managing documentation and IPP addendums, ensuring Medicaid Waiver standards, arranging placement in residential or day programs, and completing incident reports. The role requires utilization of Person-Centered Planning, strong collaboration with multidisciplinary teams, and capability to work in a hybrid office/remote/field setting with travel to Riverside East Transition area. Sign-on bonuses are offered and travel mileage is reimbursed; bilingual skills are preferred. Requires a Bachelors or Masters degree in a related field and relevant case-management experience; valid CA drivers license and insurance are required.
