Job Summary

Consumer Services Coordinator (Case Management) serves individuals with developmental disabilities in the San Bernardino Upper Desert area. Responsible for coordinating case management services, utilizing Person-Centered Planning to create and execute the IPP, generating progress reports, and conducting annual reviews. Provides advocacy to consumers and families, attends IEP/ITP/IHC meetings, coordinates access to generic resources, completes crisis intervention and incident reports, coordinates medical/educational/vocational assessments, ensures Medicaid Waiver standards, arranges placement in residential facilities/day programs, documents activities, and maintains required case-related paperwork. Supports a hybrid work environment (office, home, and field) and requires travel to consumers, families, vendors, and facilities. Maintains compliance with agency policies, participates in trainings, and supports quality assurance evaluations. Minimum qualifications include a Bachelor’s degree with one year of relevant experience OR a Master’s degree in related fields; bilingual preferred; valid CA driver’s license and liability insurance; strong communication and organizational skills.

Required Qualifications

  • Bachelor’s degree from an accredited college or university and one year of experience, including case management, in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field
  • OR Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university
  • Bilingual preferred but not required
  • Full use of an automobile, valid California driver’s license and liability insurance for the minimum amount prescribed by law or ability to provide for independent transportation
  • Must have and maintain a safe driving record
  • Ability to establish and maintain effective working relationships with others
  • Maintain good attendance and punctuality
  • Good verbal and written communication skills
  • Ability to follow oral and written direction
  • Observe safety rules and comply with IT safety protocols
  • Hybrid remote work environment familiarity
