Job Summary
Provides case coordination for consumers with developmental disabilities, utilizing Person-Centered Planning to develop and implement IPP/addendums; conducts meetings (IEP, ITP, IHC), prepares documentation, coordinates assessments and placements, ensures Medicaid Waiver compliance, and supports rights and advocacy; requires travel within San Bernardino Adult Central area and offers a hybrid work arrangement with state-required driving and bilingual capabilities preferred.
Required Qualifications
- Bachelor’s degree from an accredited college or university and one year of experience, including case management, in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field. OR Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university.
