Job Summary
Coordinate case management services for persons with developmental disabilities and their families across Riverside, Banning, and Palm Springs. Execute Individual Program Plans, conduct annual reviews, and provide advocacy by attending IEP and transition meetings. Arrange consumer assessments, manage residential placements, and complete facility audits while maintaining Medicaid Waiver standards. Document activities, secure 400 units monthly, and facilitate service purchases. Travel to vendors and facilities as needed, utilizing an automobile with a valid California driver's license. Maintain consumer files and comply with all regulatory requirements.
Required Qualifications
- Bachelor's degree from an accredited college or university
- One year of experience, including case management, in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field
- Master's degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university
- Full use of an automobile
- Possession of a valid California driver's license
- Liability insurance for the minimum amount prescribed by law
- Ability to provide for independent transportation
- Must have and maintain a safe driving record
- Ability to establish and maintain effective working relationships with others
- Ability to work cooperatively and effectively with others
- Maintain good attendance and punctuality
- Ability to follow oral and written direction
- Good verbal and written communication skills
