Job Summary
Coordinate case management services for consumers with developmental disabilities in Riverside Adult West. Utilize Person-Centered Planning to develop and execute the IPP, generate progress reports, and conduct annual reviews. Provide advocacy, attend IEP/ITP/IHC meetings, locate and coordinate generic resources, document Special Incident Reports, and ensure Medicaid Waiver standards are maintained. Arrange placement in residential facilities or training programs as clinically indicated, complete facility/agency forms, and maintain thorough case notes and 400+ Target Case Management units monthly. Travel to consumer sites in the Riverside area; hybrid work environment combining office, remote, and field work. Must hold a bachelor’s degree or higher in a related field (or a master’s degree) and at least one year of relevant experience; bilingual ability preferred; valid driver’s license and auto insurance required. Participate in mandatory trainings and quality assurance activities; support mentors and new staff; ensure consumer rights and dignity in service provision.
Required Qualifications
- Bachelor’s degree from an accredited college or university and one year of experience in intellectual/developmental disabilities, social work, special education, or a related field, OR Master’s degree in social work, psychology, public health nursing, teaching, special education, or a related field from an accredited college or university
- Bilingual preferred but not required
- Ability to establish and maintain effective working relationships; strong written and verbal communication; valid California driver’s license and liability insurance; safe driving record
