Job Summary
Coordinate case management services for persons with developmental disabilities and their families in the Riverside Adult South area, including Lake Elsinore, Wildomar, and Temecula. Utilize Person-Centered Planning strategies to create Individual Program Plans, generate progress reports, and conduct annual reviews while providing advocacy and crisis intervention. Arrange consumer assessments, coordinate residential placements, and facilitate service purchases in compliance with Medicaid Waiver standards. Complete facility audits, maintain consumer files, and document activities to meet minimum unit requirements. Organize travel to visit consumers, vendors, and residential facilities, ensuring timely completion of paperwork and adherence to quality assurance protocols.
Required Qualifications
- Bachelor's degree from an accredited college or university
- One year of experience, including case management, in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field
- Master's degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university
- Full use of an automobile
- Possession of a valid California driver's license
- Liability insurance for the minimum amount prescribed by law
- Ability to provide for independent transportation
- Must have and maintain a safe driving record
- Ability to establish and maintain effective working relationships with others
- Ability to work cooperatively and effectively with others
- Maintain good attendance and punctuality
- Ability to follow oral and written direction
- Good verbal and written communication skills
