Job Summary

Consumer Services Coordinator (Case Management) coordinates services for individuals with developmental disabilities in Riverside Adult Central, utilizing Person-Centered Planning to develop and execute IPPs, generate progress reports, and conduct annual reviews. Provides advocacy to consumers and families, attends IEP/ITP/IHC and other relevant meetings, coordinates assessments across medical, psychological, developmental, educational, vocational domains, ensures Medicaid Waiver standards, completes required documentation, and arranges placement in residential facilities or training programs as clinically indicated. Requires driving to service area (Perris, Hemet, San Jacinto, Menifee, Canyon Lake, Sun City, Quail Valley, Homeland/Romoland, Nuevo, Lake View) with mileage reimbursement; supports a hybrid in-office/remote/field work environment. Minimum education: Bachelor’s degree or Master’s degree in a related field with at least one year of experience; bilingual preferred but not required. Sign-on bonuses listed; salary is hourly with an annualized range maintained in the posting. Must maintain safety and confidentiality, participate in mandatory trainings, and collaborate with an interdisciplinary team.

Required Qualifications

  • One year of experience in case management, intellectual/developmental disabilities, social work, special education, early childhood development, or related field
  • Ability to use automobile with valid California driver’s license and insurance
  • Good verbal and written communication skills
