Job Summary
Coordinate services and supports for persons with developmental disabilities in Intermediate Care Facilities, creating Individual Program Plans, conducting annual reviews, and providing advocacy. Manage consumer assessments, placement activities, and crisis interventions while ensuring Medicaid Waiver standards and documentation are maintained. Organize travel to residential facilities and day programs, complete facility audits, and facilitate service purchases. Maintain consumer files, enter Target Case Management notes, and meet minimum unit requirements. Work in a hybrid environment, mentoring new staff, and participating in case conferences.
Required Qualifications
- Bachelor's degree from an accredited college or university
- One year of experience, including case management, in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field
- Master's degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university
- Full use of an automobile
- Possession of a valid California driver's license
- Liability insurance for the minimum amount prescribed by law
- Ability to provide for independent transportation
- Must have and maintain a safe driving record
- Ability to establish and maintain effective working relationships with others
- Ability to work cooperatively and effectively with others
- Maintain good attendance and punctuality
- Ability to follow oral and written direction
- Good verbal and written communication skills
