Job Summary
Review medication regimens, identify issues and trends, and ensure medications are appropriate, effective, safe, and used correctly. Perform Medication Regimen Reviews (MRR) as required by regulation, providing written reports to facility staff. Participate in resident assessments, care planning, and interdisciplinary team activities while assisting with compliance of federal and state laws. Coordinate controlled substance utilization reviews and provide counseling to residents and caregivers. Oversee all aspects of pharmacy services including prescribing, dispensing, storage, and disposal. Conduct exit conferences with facility administrators and assist with survey preparation. Travel as needed for facility visits and overnight stays.
Required Qualifications
- Bachelor Degree
- 3-5 years
- Must be able to lift at least 50lbs
- Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus
- Maintains required licenses or certifications in good standing with the State
- Travel as needed for facility visits, overnight stay as necessary
