Job Summary
Concierge Registered Nurse providing in-home care for older adults; conducts client assessments, education to clients and families, chronic care management, develops care plans, post-surgical and wound care, and wellness support. Collaborates with an interdisciplinary team to implement individualized care plans and provides feedback to clients, client representatives, and nurses. Requires CA RN license, 3 years of nursing experience, and ability to drive to multiple CA home-care locations (La Jolla, Encinitas, Vista).
Required Qualifications
- Current, unrestricted California registered nurse license
- 3 years of experience as a registered nurse
- Home Health experience preferred
- Proof of Negative TB result
- Physical Screening Documentation within the last year
- Active California Drivers license and car insurance
- Cell phone with Internet Access
