Job Summary
Concierge Registered Nurse providing in-home care to older adults with a focus on connection and quality care. Responsibilities include completing client assessments, educating clients and families, managing Chronic Care Management, developing care plans and goals, post-surgical and wound care as needed, and wellness support. Collaborates with an interdisciplinary team to implement individualized care plans and provides ongoing feedback to clients, family, and nursing staff. Shifts are 12-hour nights in the La Jolla/North San Diego County area with opportunities for a team-focused, supportive work environment.
Required Qualifications
- Current, unrestricted California registered nurse license
- 3 years of experience as a registered nurse
- Home Health experience preferred
- Proof of Negative TB result
- Physical Screening Documentation within the last year
- Active California Drivers license and car insurance
- Cell phone with Internet Access
